1. Reviewing and accepting our offer
If you haven't already done so, the first thing you need to do is read through your contract of employment and the information in What you need to know.
2. Statement of Health
Please follow this link: https://portal.healthmanagement.org.uk/EmployeeLogin.aspx and complete the short questionnaire provided by our Occupational Health advisers, Health Management ltd.
The purpose of this questionnaire is to see whether you have any health problems that could affect your ability to undertake the duties of the post that you have been offered or place you at any risk in the workplace. Health Management ltd may recommend adjustments or assistance as a result of this assessment to enable you to fulfil the role.
Your information will be treated as confidential and will not be given to anyone else without your written permission.
Failure to complete this form may delay your start date with Zurich.
3. Pre-employment referencing
Zurich requires pre-employment referencing from all new employees. This task is carried out in partnership with The Security Watchdog Screening Bureau (TSW). The process will take around 30 minutes to complete and before you start you will need to have details of your employment history, national insurance number and education achievements to hand.
You will have received an email link from TSW asking you to complete an online pre-employment application form.
If you have not yet completed the application form, please can you action this as soon as possible. Failure to complete this form may delay your start date with Zurich.
4. Your information
In order for you to formally agree our terms and conditions of employment and provide us with the relevant information, before you join, we need you to complete the following forms which have been included in your “E Offer” letter. You will need to complete the attachments for:-
- Personal Details
- Conflict of interest
- External Engagement Notification
- CB1 F&P Questionnaire (if required)
To do so click on the form in Acrobat or Adobe Reader and then 'submit' to save locally for submission via email or alternatively print and return your completed forms to:- Zurich Resource Centre, TC1 2E, Newbridge Square, SWINDON SN1 1HN.
Once we have received your Personal Details form and completed the pre-employment checks, including referencing and Health Questionnaire, we will contact you to arrange a suitable date for you to commence your role. Please email all the completed forms to MyEmployment@uk.zurich.com. In the subject heading of your email back to us, please quote your contract reference number (this can be found in the subject header of this email) and in the general content of your email please state that you are accepting receipt of the Contract of Employment.
